Population health management is a critical component of integrated care. In the AIMS model it is referred to as the “Collaborative Care” model.
Click the “Collaborative Care” tab and read about the “Core Principles”, “Team Structure”, “Care Manager”, “Dollars and Sense”, “Resource Library” (amazing!).
Click the “Implementation Guide” and review steps 1 through 5.

Why is this scavenger hunt important?
In this course you will be required to complete a proposal for an integrated health care program in a primary care clinic settings. The proposal will require that you address clinical, operational and financial strategies. In addition, it will require that you incorporate a population health management component. The AIM collaborative care resources will prove extremely valuable as this program is population based, and the resources cover clinical, operational and financial topics.

Pick one topic or section from the AIMS website that you were most interested in. It may be a new topic that you had little background on while working in Management at Blue Cross and Blue Shield. It may be details for a clinical, operational or financial resource that is related to work you have done or that you have been searching for. For your post please indicate:
